It's important to fully understand breast implant safety. If you're thinking about Motiva breast implants, you may have heard about their smooth surface, responsive silicone gel, and how natural they can feel. However, these features do not eliminate the risks that come with breast augmentation surgery. Motiva implants are FDA-approved for certain patients, include the required boxed warning, and require ongoing monitoring.
For the right patient, Motiva breast implants are FDA-approved and have clinical evidence showing they are reasonably safe and effective. However, they are still medical devices and not meant to last a lifetime. Complications can happen, more surgery might be needed in the future, and rare cancers have been reported in the scar tissue around breast implants.
The best way to stay safe is to make an informed decision. This means knowing what FDA approval covers, what is still being studied over time, and how your own anatomy, implant size, surgery plan, and follow-up care can affect your personal risk.
Are Motiva Implants FDA-Approved?
Yes. The FDA approved Motiva SmoothSilk Round and Motiva SmoothSilk Round Ergonomix silicone gel-filled breast implants on September 26, 2024. The approval covers primary breast augmentation in women aged 22 or older and revision surgery intended to correct or improve a previous breast augmentation result.
FDA approval means the agency has reviewed how the device is made, tested, labeled, and the clinical evidence, and decided that the likely benefits are greater than the likely risks when used as intended. This does not mean the implant is risk-free, will last forever, or is better than all other silicone options.
The FDA decision relied substantially on three-year findings from the Motiva core study. Longer follow-up is still underway. Establishment Labs is required to continue following the original study participants for 10 years and to conduct additional post-approval research examining local complications, rupture, reoperation, implant removal, systemic symptoms, reproductive outcomes, and cancer.
What Makes Motiva Breast Implants Different?
Motiva implants are form-stable silicone implants with a silicone elastomer shell and cohesive silicone gel. The approved devices use a controlled SmoothSilk surface architecture. FDA labeling describes an average surface roughness of four microns and identifies the devices as smooth implants.
The two approved styles have different looks. Motiva Round implants use a firmer gel and may be a good choice for patients who want more fullness at the top of the breast. Round Ergonomix implants have a slightly softer gel. They look round when you are lying down and take on a more natural slope when you are standing. However, how the implant behaves does not guarantee natural movement or that the result will look like natural breast tissue for everyone.
How natural the implants feel and whether rippling is visible depend on more than just the implant itself. Your own breast tissue, skin quality, chest size, where the implant is placed, the size of the implant, and the surgical technique all play a role. A smaller implant is not always safer, and a larger one is not always wrong. The best size and shape are the ones that fit your body and your goals.
Some Motiva implants have an optional radio frequency identification microtransponder that stores a unique serial number for tracking the device. This helps with identification, but it does not treat any condition or prevent problems like rupture, capsular contracture, or other complications.
Can Motiva Implants Cause Cancer?
There is a possible cancer risk that must be discussed before any breast implant surgery. The most established implant-associated malignancy is breast implant-associated anaplastic large cell lymphoma, or BIA-ALCL. It is a rare cancer of the immune system that usually develops in the fluid or scar tissue surrounding an implant. It is not breast cancer and generally does not begin in natural breast tissue.
The FDA's boxed warning says that breast implants have been linked to BIA-ALCL, and this cancer happens more often with textured implants than with smooth ones, though exact rates are unclear. Motiva SmoothSilk implants are smooth, not the macrotextured type that led to the 2019 recall of some Allergan BIOCELL products. Having a smooth surface may matter when comparing implants, but it does not mean there is no cancer risk.
No cases of BIA-ALCL were found among people in the Motiva core study during the first three years. This is reassuring for that time frame, but the study was not big or long enough to rule out rare problems that can show up years later. Motiva was only approved in the U.S. in 2024, so more long-term data is still being gathered.
The FDA has also received reports of squamous cell carcinoma, various lymphomas other than BIA-ALCL, and other rare tumors in the capsule around breast implants. Reports have involved both smooth and textured implants and both saline and silicone implants. The FDA considers these occurrences potentially rare, but their incidence, causes, and risk factors remain unknown. Current evidence does not show that Motiva implants uniquely cause these cancers.
If you notice ongoing swelling, a new lump, pain, hardening, unevenness, or a major change in your breast after the initial healing period, contact your plastic surgeon right away. These symptoms can have many causes, but they should be checked. The FDA does not recommend removing implants in patients without symptoms just because of concerns about BIA-ALCL, squamous cell carcinoma, or other lymphomas linked to the capsule.
What Other Risks Should Patients Understand?
Motiva breast implants share common risks with other silicone breast implants and with breast surgery itself. Possible complications include:
- Capsular contracture, in which scar tissue tightens around the implant and may cause firmness, pain, or visible distortion
- Implant rupture or silicone gel bleed through an intact shell
- Implant malposition, rotation, asymmetry, or changes in breast implant shape
- Visible rippling, palpability, or an unsatisfactory aesthetic appearance
- Infection, bleeding, hematoma, seroma, delayed healing, or abnormal scarring
- Changes in nipple or breast sensation
- Breast pain, tissue thinning, or interference with breastfeeding
- Reoperation, implant removal, or replacement
- Anesthesia and general surgical risks
The three-year core study included 451 primary augmentation patients and 109 revision augmentation patients. The estimated risk of any complication, including reoperation, was 8.4% after primary augmentation and 28.4% after revision augmentation. Reoperation rates were 6.1% and 25.8%, respectively. Severe Baker grade III or IV capsular contracture was estimated at 0.5% after primary augmentation and 6.7% after revision augmentation.
These numbers are based on study groups and do not guarantee what will happen for any one person. They also should not be used to say that Motiva has a lower rate of capsular contracture than other implants unless there is a direct comparison. Patients having revision surgery often start with scar tissue, stretched tissue, implant malposition, or other issues that make their surgery more complicated.
What Is Breast Implant Illness?
Breast implant illness is a patient-used term for systemic symptoms reported after breast implants. These may include fatigue, brain fog, muscle or joint pain, headaches, hair loss, anxiety, rash, and other symptoms. BII is not currently a single formal diagnosis with one confirmed test or cause.
The FDA requires that patients be informed that these symptoms have been reported with breast implants. It is not clear how likely any one person is to experience them. Some people feel better after their implants are removed, but others do not. If you have these symptoms, you should get a careful medical evaluation, since they can also be caused by autoimmune, hormonal, infection, nerve, or other health problems. Choosing Motiva instead of traditional silicone implants does not remove the risk of breast implant illness.
How Long Do Motiva Implants Last?
Motiva breast implants do not have a set expiration date. They are called lifetime devices only because they stay in place until they are removed. The FDA makes it clear that breast implants are not meant to last a lifetime.
You do not need to replace your implants every 10 years if they are intact, feel comfortable, and you are happy with the results, but you should expect that you may need another breast surgery at some point. The chance of complications, removal, or replacement usually increases the longer the implant is in your body.
What Monitoring Is Needed After Surgery?
Long-term care has several parts:
- Attend every scheduled postoperative visit and continue periodic clinical follow-up after healing.[cite: 1]
- Become familiar with the normal look and feel of your breasts, then report persistent swelling, pain, a lump, hardening, asymmetry, or a change in implant position.[cite: 1]
- For silicone breast implants, obtain the first ultrasound or MRI to screen for silent rupture five to six years after surgery, then repeat imaging every two to three years. MRI is recommended when symptoms are present or ultrasound findings are uncertain.[cite: 1]
- Continue age- and risk-appropriate breast cancer screening. Implant-rupture imaging does not replace mammography. Tell the imaging center that you have implants so trained personnel can use implant-displacement views and other appropriate techniques.[cite: 1]
- Keep your patient device card and implant details. Device name, implant size, serial number, surgery date, and surgeon should remain in your medical records.[cite: 1]
A silicone implant can rupture without any obvious signs, so neither you nor your surgeon may notice just by looking or feeling. New lumps, changes in shape, swelling, burning, numbness, or hardening can be signs of rupture, but imaging tests are needed to check the implant properly.
Who Should Not Receive Motiva Implants?
According to the approved labeling, breast augmentation with Motiva implants should not be performed in a patient who has an active infection anywhere in the body, untreated breast cancer, a precancerous breast condition, or who is pregnant or nursing. Motiva augmentation is FDA-approved for women ages 22 and older.
If you smoke, use nicotine, have poorly controlled diabetes, bleeding problems, take immune-suppressing medications, have had radiation, or have conditions that affect healing, you will need a personal assessment. It's also important to have realistic expectations. Breast augmentation cannot promise a certain cup size, a natural feel, a subtle change, or that you will be completely satisfied.
How Does Surgical Planning Affect Patient Safety?
Device selection is only one part of safe breast augmentation surgery. A board-certified plastic surgeon should evaluate breast tissue, skin quality, chest width, asymmetry, nipple position, and the amount of coverage available over the implant. These findings guide implant size, profile, pocket placement, incision choice, and whether a breast lift is needed.
Good planning can help prevent some problems, but even the best surgery cannot remove all risks. Using sterile techniques, creating the right pocket, controlling bleeding, handling the implant properly, and giving clear recovery instructions are all important. Motiva stands out because of its SmoothSilk surface, the choice between Round and Round Ergonomix styles, its gel features, and optional electronic identification. Traditional silicone implants have their own gel types, shapes, sizes, and long-term data. The best implant is the one that matches your body, your goals, and what tradeoffs you are comfortable with.
Be cautious about claims of advanced safety, smaller incisions, natural movement, or being the best. A design feature can be important, but that does not always mean it leads to better results. You deserve a comparison of implants based on approved information, real evidence, and your own anatomy.
